In order to qualify for multiple restoration rebates (up to a maximum of $600), these restorations must be completed during the same appointment, within the same dental quadrant, and must be restored during the same visit. At the completion of your appointment, you must have your Dentist sign the rebate form and verify that your treatment was paid in full. Rebate form must be post marked within 30 days of treatment performed. Upon verification your rebate will be sent to you within 90 days of receipt of your rebate request.
